General Liability Release

I acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own acknowledge further, that there may be other risks not known or not reasonably foreseeable at this time. I assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death. Release, waive discharge and covenant not to sue the HAWAII UNITED FC or their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leaser’s of premises used to conduct the event, all of which are hereinafter referred to as “Releasees” from demands; losses or damages on account of injury, including death or damages to property, caused or alleged to be caused in whole or in part by the negligence of the “Releasee” or otherwise. Part 2: Publication/Audio/Video Release I hereby give my permission to HAWAII UNITED FC to create or use digital or print media of my child, which may include, but not limited to, video and audio recordings, photographs, and images for the purpose of promoting HAWAII UNITED FC. I understand that the distribution of these media may include print, online or digital. I understand that there will be no compensation, financial or otherwise, for the use of these media. I understand that permission is granted for the life of the media. Part 3: Covid 19 Liability Release I agree and acknowledge the following: 1. I will comply with all safety protocols that have been or will be implemented HAWAII UNITED FC. 2. My child is not experiencing any symptoms of illness including a fever, cough, or shortness of breath. 3. My child has not been diagnosed with COVID-19. 4. I do not believe that my child has been exposed to a person with a confirmed or suspected case of COVID-19. 5. If my child develops any COVID-19 symptoms, or if my child has a suspected or diagnosed case of COVID-19, I agree that my child will not attend or participate in any HAWAII UNITED FC activities. 6. I agree to notify HAWAII UNITED FC immediately if I believe that my child is experiencing any symptoms of COVID-19 and/or if my child has a suspected or diagnosed case of COVID-19. 7. I agree and acknowledge that my child’s participation with HAWAII UNITED FC activities may involve inherent danger and risk, including, without limitation, the risk of exposure to, and/or illness resulting from, the novel coronavirus (“COVID-19”), and I knowingly and voluntarily assume all such risks and release HAWAII UNITED FC from any liability whatsoever.
